The ILIOTIBIAL BAND (IT band) syndrome is the most common cause of knee and hip pain in cyclists. This band, Ron_Bike.jpg (36978 bytes)which runs on the outside of the leg from the hip to the knee, is a thick fibrous band of tissue. The band can become tight and pain is caused when the band rubs over the bones of the hip (greater trichinae) and/or the knee (lateral epicondyle). Cycling, Basketball, Volleyball, Running - all of these activities involve repetitive running and/or jumping and could cause a very painful knee condition called patellar tendinitis, Iliotibial Band Syndrome or Patellofemorial known as "cycling's knee".  The act of jumping, especially, puts a great deal of stress on the patellar tendon, which attaches to the bottom of your kneecap.  The force of hitting the ground repeatedly could cause the tendon to become inflamed and even begin to tear.

Iliotibial band syndrome (cycling's knee) is usually felt in the front of the knee just below the kneecap and may be felt during or after vigorous running or jumping. You may also experience discomfort when you sit down and extend your leg straight out.

The cyclist can look at his knee and find a few things, which could suggest the existence of Iliotibial band syndrome as the cause of pain.

1. Look at the quad muscle just above the knee on the inside of the leg known as the vmo. Frequently with cycling's knee, there is poor development or weakness of this portion of the quad muscle.

 2. While sitting in a chair extending the leg, watch the patella move. Often with Patellofemoral pain, when the leg is almost fully extended, the kneecap will move toward the outside of the leg. Patella movement is the result of a tracking abnormality of the patella, which predisposes to cycling's knee pain. Patella tracking to the inside causes the same syndrome in a few cycling's.

3. X-rays are rarely abnormal but can offer evidence that other knee problems are not causing the pain. X-rays of the patella may show that it is deviated to the outside.
