Clinical gout comprises a heterogeneous group of disorders characterized by deposition of monosodium urate crystals in the joints and tendons. Gout progresses through four clinical phases: asymptomatic hyperuricemia, acute gouty arthritis, intercritical gout (intervals between acute attacks) and chronic tophaceous gout. Demonstration of intra-articular monosodium urate crystals is necessary to establish a definitive diagnosis of gouty arthritis. Asymptomatic hyperuricemia is common and should not ordinarily be treated. Nonsteroidal anti-inflammatory drugs (NSAIDs) are the treatment of choice for acute attacks of gout in most patients. NSAIDs should be used sparingly in elderly patients and should be avoided in patients with renal disease and peptic ulcer disease, and in those receiving concurrent systemic anticoagulation. Corticosteroids are a valuable treatment option for patients in whom NSAID therapy is contraindicated. Acute gouty arthritis and chronic gout require different treatment strategies.

Gout is a condition characterized by the deposition of monosodium urate crystals in the joints or soft tissue. The four phases of gout include asymptomatic hyperuricemia, acute gouty arthritis, intercritical gout and chronic tophaceous gout. The peak incidence occurs in patients 30 to 50 years old, and the condition is much more common in men than in women. Patients with asymptomatic hyperuricemia do not require treatment, but efforts should be made to lower their urate levels by encouraging them to make changes in diet or lifestyle. Acute gout most commonly affects the first metatarsal joint of the foot, but other joints are also commonly involved. Definitive diagnosis requires joint aspiration with demonstration of birefringent crystals in the synovial fluid under a polarized light microscope. Treatment includes nonsteroidal anti-inflammatory drugs (NSAIDs), colchicine, corticosteroids and analgesics. In patients without complications, NSAID therapy is preferred.

 

Epidemiology

Gouty arthritis is the most common form of inflammatory joint disease in men older than 40 years.1 The National Health Survey (1983 to 1985) determined the prevalence rate of self-reported gout to be 13.6 cases per 1,000 men and 6.4 cases per 1,000 women. These numbers reflect an approximate threefold increase in the prevalence of gout since 1969.2 In contrast, cases of physician-diagnosed gout suggest a consistently lower prevalence rate--5.0 to 6.6 cases per 1,000 men and 1.0 to 3.0 cases per 1,000 women.3,4

Gout affects 2.1 million Americans; most are men 40 to 50 years of age.  The most common symptom of gout is sudden, severe attacks of pain and swelling in some joints. It usually affects one joint at a time, especially the joint of the big toe, but can also affect the knee, ankle, foot, hand, wrist and elbow.

Gout, a condition that manifests itself as recurrent attacks of acute arthritis, may become chronic and deforming.  About 95% of patients with this foot disorder are men, usually over 30.  Alcohol or dietary excesses as was once thought do not cause it, but there seems to be a hereditary factor involved.  The presence of increased uric acid in the body distinguishes gout from other forms of arthritis, although hyperuricemia alone, which often occurs in complete absence of gout, is not thought to be the sole causative factor.  Gout usually begins with an acute attack of pain, inflammation, extreme tenderness, and redness in the affected joint—often the big toe, foot and sometimes the ankle or knee.  After repeated attacks of gout the disease can cause the deposition of sodium urate crystals in the tissues about the joints, causing stiffness and deformity.

The product has proven effective in relieving pain, swelling and stiffness from gout in muscles and joints. There have been no complaints about allergic reactions. The FDA laboratory in Seattle, Washington tested the product to see if any of the semiconductors were absorbed into the skin.  This test concluded the elements were not absorbed into the skin.

The Health Sciences Department or Medical Schools at five major universities have been involved in research.  A double blind pain study was conducted at a western university and the results stated, "Seven Bsktball2.gif (2375 bytes)days- 33% of control group subjects had again slight improvement. 91% of the treatment group subjects had great to total improvement.  Significance is .004.”  An anti-inflammatory study by a southwest university concluded, "These data indicate that semiconductors are an active topical anti-inflammatory agent when impregnated on cloth or on tape.”  Another study by the same university summarized by stating, "There does not appear to be any consistent effect of the semiconductor patches on blood flow or local temperature in resting subjects.  In terms of the recovery from local heating, most subjects show nearly identical responses at site with and without the semiconductor patch/pellet combination.”  A third study on muscle relaxation states, "In several experiments, we have been able to record a reduction of force through the motion cycle after the application of semiconductors over the bellies of the planar flexor muscles.  This effect occurs within very few minutes of application of semiconductors."

The basic science for the mechanism has not been established. A number of hypotheses have been advanced.
